History of PGT-A Development

Traditionally, alongside human visual morphology screening, embryo quality has been graded through PGT (Pre-Implantation Genetic Testing. This process maps millions of DNA pieces to their correct place on the chromosome to identify the chromosomal pattern. This process has developed from the 1990s through the 2000s, 2010s and 2020s. Advancements included FISH 1993, Metaphase CGH 1999, aCGH in 2008, and NGS in 2014. These advancements have allowed embryologists to detect chromosomal anomalies with increasing accuracy, including deletions and duplications.

  2. Embryo Comparison:
    The system evaluates embryos using advanced image recognition, comparing their microscopic features with those of embryos that have led to successful pregnancies.
  3. AI-Driven Grading System:
    Based on this analysis, the AI assigns a grading score to each embryo, predicting which ones are most likely to implant successfully. This grading is independent of PGT (Preimplantation Genetic Testing), which ensures only embryos with the normal male/female karyotypes of 46XX and 46XY are transferred, and all other karyotypes are not transferred, offering an additional layer of precision for embryologists.
  4. Supporting the Embryologist’s Decision:
    The embryologist then reviews both the PGT results (which can help identify abnormal extra or missing chromosomes) and the AI’s grading recommendations to decide which embryos to freeze or transfer, combining technology and human expertise to make the most informed decision possible.

A 2023 study published in the Journal of Assisted Reproduction and Genetics by researchers Julia Buldo-Licciardi, Michael J. Large, David H. McCulloh, Caroline McCaffrey, and James A. Grifo, stated that Utilisation of standardised preimplantation genetic testing for aneuploidy (PGT-A) via artificial intelligence (AI) technology is correlated with improved pregnancy outcomes in single thawed euploid embryo transfer (STEET) cycles. The study compared the use of AI in analysing embryos with trained scientists. The AI group had a positive pregnancy test rate of 83% compared to 81% for the scientists. The early pregnancy loss rate with AI was 5% compared with 12% for the scientists. The percentage of embryo transfers that had a live birth with AI was 70%, compared to 61% without AI.
